Register for an appointment



Register on Patient Portal

Follow the link below to go to the patient portal.

Once there click on the bottom right corner where it says “register” and follow the instructions to create your account.



Schedule Appointment

After Registering we take the info from your account to call you to make your appointment.



Fill out Questionnaire

Once you get the appointment you will have to go back on to the Patient Portal and with the credentials sent to you log on and complete the questionnaire.


